Job Summary The Nurse Pharmacist is responsible for providing high-quality nursing care and pharmaceutical services to ensure the effective delivery of integrated primary healthcare and nutrition services under the Healthcare and Nutrition Support for Vulnerable Afghan Communities (HNS-VAC) project. The position supports maternal, newborn, and child health services by assisting with patient assessment, medication dispensing, health education, infection prevention and control, and the management of pharmaceutical and nutrition commodities. The Nurse Pharmacist ensures the availability and rational use of essential medicines, maintains accurate pharmacy and patient records, and provides counseling on medication adherence and healthy practices. Working closely with the Medical Doctor, Midwife, Nutrition Counselor, and other health staff, the Nurse Pharmacist contributes to improving the health and nutrition status of vulnerable women and children while ensuring compliance with Ministry of Public Health (MoPH), organizational, and donor guidelines. Duties and Responsibilities · To work closely with project team for proper implementation of the project · Dispensed prescribed medicines accurately and provided patients with clear instructions on dosage, administration, storage, and possible side effects. · Assisted in patient assessment by measuring vital signs, documenting medical histories, and supporting clinical consultations. · Administered medications, injections, intravenous (IV) fluids, and vaccinations according to physician orders and nursing protocols. · Maintained pharmaceutical stock by monitoring inventory levels, expiry dates, and storage conditions using FEFO/FIFO principles. · Ensured the availability of essential medicines and medical supplies through timely stock requests and accurate record-keeping. · Provided health education and counseling to patients on medication adherence, disease prevention, nutrition, hygiene, and healthy lifestyles. · Supported infection prevention and control (IPC) practices by maintaining a clean and safe clinical environment and following standard precautions. · Assisted in emergency care, wound dressing, first aid, and stabilization of critically ill patients before referral when required. · Maintained accurate patient records, pharmacy registers, and daily/monthly reports in accordance with organizational and Ministry of Public Health guidelines. · Collaborated closely with physicians, pharmacists, laboratory staff, and other healthcare professionals to ensure high-quality patient care. · Participated in health campaigns, immunization activities, community outreach, and continuous quality improvement initiatives. · Adhered to professional ethics, patient confidentiality, and organizational policies while delivering compassionate and patient-centered care. · Update daily stock card, register book, HMIS tools related to the nurse and pharmacy. · To identify and report any outbreaks to the line supervisor, properly manage the outbreaks and provide the final report. · To identify and report immediately any problem, which interfere in the delivery of services or in achieving the program objectives to the line supervisor. · Participate regularly in staff meetings and monthly technical meetings. · To undertake other tasks as requested by the line supervisor. Job Requirements PERSON SPECIFICATION It is essential that the post holder shows a good understanding and sympathy with Islamic values and principles as well as commitment to Islamic Relief Worldwide vision, mission and Protection as well as PSEA. Essential: Knowledge, Skills and Qualifications (Bullet Points) · Diploma (14 Grade) in nursing or pharmacy from a recognized university. · Successfully passed the National Exit Examination. Minimum of 2–3 years of relevant professional experience in nursing and/or pharmacy. Strong verbal and written communication skills. Excellent interpersonal and teamwork skills. Proficient in computer applications, including Microsoft Office (Word, Excel, PowerPoint, and Outlook). Fluent in Pashto, Dari, and English (spoken and written). Willingness and ability to travel frequently to field locations as required.
